Digital Documents Outsourcing Services A. Surveys, Questionnaires and Product Registration Form Design - We will use your current form (or a Word Processor mock-up) and design a form which is both aesthetically pleasing and also ensures the highest degree of accurate automated data capture. Question Structuring - during the Form Design process we will provide your staff with suggestions on how to phrase questions that are both easy for an end user to understand and will accurately depict their feelings about your products and services. Camera Ready Artwork - we will provide 2400 DPI output for your selected printing company to reproduce. Form Validations - we will check that required fields are filled in, that database look-ups (part numbers, etc.) are correct or any conditional field logic is met (i.e., if question 1 is answered yes, questions 2 and 3 must also be filled in). Scanning - we will scan and interpret any forms returned via mail on our high speed scanners. Fax Broadcasting - by providing us with your logo and a list of your Client's fax numbers we can design a custom Word document to be sent along with your Survey. The documents will be sent automatically to each of your intended recipients and will give the appearance that you sent each directly from your facility. Fax Receiving on 1-800 lines - We can dedicate up to 24 lines for your Clients to return their completed forms and our system will immediately interpret the forms upon receipt. Data Export Format - we will export your interpreted data to whatever format you require (Excel, Access, Foxpro, dBase, ASCII, SQL Server, etc.). Reporting - if you require analysis services we will produce reports based on your unique requirements. Costs: Form Design - approximately $350.00 per page (depends on number of fields, etc.) We will be happy to provide you with a No Obligation Quote. Simply fax us your current form(s). Fax Broadcasting - approximately 35 cents per page (varies based on quantity). Fax Receiving on 1-800 lines - approximately 50 cents per page (varies based on quantity). Form Interpretation, Validation and Data Export - approximately 25 cents per page (varies based on quantity and number of field validations). Reporting - varies based on number of reports and requirements. B. Document Scanning, Indexing and Storage onto CDs Key Field Requirements - we will work with you and your staff to determine the most effective means to query and find documents based on unique key fields (such as Name, SSN, Account Numbers, Date, etc.). Document Preparation - we will prepare your documents for scanning by removing paperclips, staples and smoothing out dog ears and folds. CD Authoring - we will burn your images and corresponding indexes onto standard CD-ROMs. Each CD-ROM contains an intuitive query interface with the ability to View, Fax (if workstation has a Fax Modem or is connected to a Fax Server) and Print.
